Registered Nurse, Preadmission Screening Unit (PASU) Transfer Coordinator

The function of the PASU is to coordinate acute to acute inpatient transfers from other VA's and community hospitals. The requires close collaboration with patient care providers outside of and within VAPAHCS, Business Administration Service (BAS), and Beneficiary Travel to ensure the smooth and safe transfer of all eligible Veterans, and to ensure that all admissions receive the appropriate level of care. The major duties and responsibilities include, but are not limited to:
Responsible for screening requests for admission to the appropriate level of care at VAPAHCS. Facilitates a thorough medical review from the referring medical facility and works with the Nursing Supervisor to secure a bed for the patient. Prioritizes transfers according to eligibility in collaboration with the accepting MD when beds are limited. Presents current medical information to the attending physician of the appropriate service to arrange doctor-to- doctor review between the sending and receiving physicians. Requests medical records for review for consideration of transfer. Communicates with the sending case manager to ensure all the pertinent medical documentation accompanies the patient (reports, labs, radiology, discs, etc.). Responsible for coordination with the VA Travel Office the ordering of an ambulance to transport the patient to VAPAHCS, if applicable. Interacts with Non-VA Care staff and uses the Fee Basis Claims Systems (FBCS) software program to determine eligibility and medical necessity of VA patients hospitalized in the community. Reports on the electronic FBCS claims forms for payment of patient care services. Coordinates continuous communication between the sending facility and the receiving facility (nurse-to-nurse and doctor-to-doctor reports are given), the Nursing Supervisor, the Administrative Officer of the Day (AOD), the travel clerk, and the receiving ward. Records the Transfer Coordinator-Nursing Supervisor note in the patient's chart. When medical reviews are faxed, scans them into the patient's chart and assigns PACT Team Care Manager as an additional signer, so the team is aware the patient was hospitalized in the community when he/she returns to clinic. Maintains data bases for tracking workload and benchmarking for performance improvement projects. All other duties as assigned Work Schedule:

United States Citizenship. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency, and accredited by the National League of Nursing Accreditation Commission (NLNAC) which changed to the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE) at the time the program was completed; OR the completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005/27, Appendix G6). OR In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration (i.e., licensure) as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or the District of Columbia. Five (5) years of RN experience in Medical-Surgical or Critical Care area. Current BLS provider card issued by American Heart Association (AHA) or Military Training Network (MTN). Two (2) years of RN Case Management experience. Current Advanced Cardiovascular Life Support (ACLS) provider card. National Certification in Medical-Surgical Nursing recognized by ANCC. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (2 hours); reaching above shoulder; use of fingers; both hands required; walking & standing 8 hours - intermittently; repeated bending; both legs required; ability for rapid mental& muscular coordination simultaneously; near vision correctable at 13 to 16 to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
