The ACOS for Community Care Integration is part of a triad leadership structure at VAPAHCS. As part of the triad with the Ambulatory Care Chief Nurse and the Chief of Business Administration, the ACOS works closely with other clinical dyad teams to meet VAPAHCS' mission and vision. This includes improving the quality and efficiency of non-VA care, as well executing initiatives and goals determined by executive leadership. The dyad teams are responsible for oversight and management of their respective functional areas, and work in collaboration with all service chiefs. The Community Care Integration triad ensures transparency by using the SIM model and the RACI format to clarify roles and responsibilities. The incumbent has responsibility for planning, developing, and directing the administrative and operational activities for Community Care Integrations services at all VAPAHCS sites of care, and is accountable for the quality, safety, efficiency, access to care, and patient experience in all Community Care settings. Responsibilities to include:
Performance Measures Program Development Resource Management Accreditation Quality Assurance Direct Supervision of Staff Budgets Equipment (major and minor) Maintenance Contracts Personnel Evaluations Performance Reviews Credentialing Design and Implementation of Goal-Sharing Initiatives Clinical workload Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. Moderate lifting (15-44 lbs); moderate carrying (15-44 lbs); straight pulling (1 hour); pushing (1 hour); reaching above shoulder; use of fingers; both hands required; walking (5 hours); standing (4 hours); kneeling (1 hour); repeated bending (4 hours); both legs required; ability to perform rapid mental and muscular coordination simultaneously; near vision correctable at 13 to 16 Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; hearing (aid permitted); emotional/mental stability.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
