Staff Nurse (PACT Program Leader) RELOCATION INCENTIVES/MOVING EXPENSES ARE NOT AUTHORIZED.
The incumbent of this position will work for the VA Palo Alto Health Care System as an RN PACT Program Leader.
This person directs and provides comprehensive preventative and wellness care, as well as the spectrum of acute, episodic, chronic and end-of-life care to Veterans.
The RN collaborates for the improvement of patient care outcomes in the Patient Aligned Care Team.
This person executes position responsibilities that demonstrates clinical leadership,, experience, and creative approaches to the management of patient care across microsystems.
The RN serves as a mentor and consultant to healthcare staff involved in patient care.
The major duties and responsibilities include but are not limited to:
Provides leadership in implementation and evaluationof guidelines, professional standards and policies that impact patient care in the Patient Aligned Care Team.
Develops and promotes systems to improve access and continuity of care.
Uses advanced clinical knowledge and critical thinking skills to mentor staff in planning, implementing and evaluating interventions that improve patient outcomes.
Designs and provides age and population specific health promotion and risk reduction strategies.
Translates evidence-based research into practice to ensure that patients benefit from the latest innovations in nursing science.
Manages patients in transition between levels of care.
Serves as an expert resource to implement and teach skills, including motivational interviewing to promote patient self-management toward patient-driven holistic care plan for life.

Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
